Description: The RN implements professional nursing practice centered on the patient as a whole person. This practice focuses on the creation of a caring environment centered on healing and wellness. Nursing practices are founded on evidence, current research, and relevant policies and procedures. Clinical decision-making incorporates all phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through collaboration among nursing staff and other disciplines. The RN coordinates and provides direct patient care activities on the unit. This includes nursing rounds, assessments, and treatments. The RN facilitates admission, transfer, and discharge activities. Patient and family-centered education is a key component of care. The RN develops individualized treatment planning as applicable. Responsibilities include performing and documenting assessments according to scope of practice and hospital policies. The RN considers the patient’s culture, age, and developmental level. Identifying risk factors and presenting complaints is an essential task. The RN collects both objective and subjective data. A problem list is developed, prioritizing patient needs with input from the inter-professional team. Actions and interventions are designed relevant to the patient’s culture and age. Desired outcomes are determined in coordination with the patient and family. The RN implements patient care therapies within scope of practice. Procedures, treatments, and processes are explained to patients and families. The RN initiates actions to alleviate patient stress and discomfort. Changes in patient condition are recognized and communicated. Ongoing assessments are conducted and documented according to established standards. The RN delegates tasks appropriately and oversees performance by team members. Monitoring the accuracy and completeness of care provided by others is crucial. Educational needs of patients are identified and addressed. Facilitating the discharge process through timely execution of discharge orders is necessary. The RN provides input into patient/nurse assignments based on available staffing and patient acuity. Active participation in unit-based activities supports organizational values. A positive group dynamic and healing environment is promoted through words and actions. The RN assures nursing excellence according to accepted standards and evidence-based practice. Participation in quality improvement activities and core measures is expected. A holistic perspective regarding patient care is employed.
