Work Environment This role is primarily onsite at our rehabilitation facility in Cortlandt Manor, New York, a region known for its scenic beauty and community focus. The facility features a collaborative atmosphere where interdisciplinary teams coordinate to deliver holistic patient care. You will have the opportunity to work closely with a variety of healthcare professionals, including physicians, nurses, and occupational therapists, ensuring a well-rounded approach to patient recovery. Key Responsibilities Conduct thorough evaluations of residents to assess their physical capabilities and determine individualized treatment needs. Design and implement tailored treatment plans focused on enhancing residents' strength, mobility, and functional independence. Administer therapeutic exercises and utilize techniques such as manual therapy and electrotherapy to address specific physical impairments. Monitor and document progress meticulously, modifying care plans as necessary based on ongoing assessments and feedback. Engage in regular collaboration with an interdisciplinary team, contributing to a unified strategy for patient care that encompasses all aspects of health. Provide education to residents and their families regarding home exercise plans and strategies to maintain and improve physical health outside of therapy sessions. Uphold compliance with state and federal regulations while maintaining the highest professional standards in all interactions. Patient Population / Client Focus The primary focus of this role is the geriatric population, including residents recovering from surgeries, acute injuries, or managing chronic conditions. You will be expected to understand their unique physical therapy needs and to employ compassionate methods that accommodate their age and health status. Qualifications Must possess a current state licensure as a Physical Therapist. A Bachelor's Degree in Physical Therapy is required, though advanced degrees such as a Master's or Doctorate are preferred. Demonstrable strong communication, organizational, and interpersonal skills are essential to thrive in our collaborative environment. Ability to work effectively within a team and maintain open lines of communication between team members. Preferred Experience Two years of experience as a Physical Therapist is required. Experience in a skilled nursing facility and working specifically with the geriatric population is highly desirable and advantageous.
