Clinical Specialist Pharmacist - Pharmacy Administration - TVH-Seville Hospital Drive

Job Summary Responsible for managing the medication use process - prescribing, dispensing, administration, and monitoring - throughout the medical center, and for applying advanced knowledge about drugs and drug therapy to the care and treatment of patients. Provide advanced clinical services independently and promotes pharmacy department clinical development. Responsibilities
• Monitor all patients in assigned practice area for drug-disease state interactions (renal failure, ESRD, CHF, liver disease, etc.).
• Monitor all patients for drug-drug and drug-nutrition interactions.
• Evaluate drugs that require pharmacokinetic monitoring
• Identify opportunities for pharmaceutical streamlining and contacting prescriber.
• Perform requested consults (i.e., pain management, parenteral/enteral nutrition, etc.).
• Actively participate in assigned collaborative practice.
• Actively participate in assigned pharmacy and therapeutic/formulary management initiatives
• Precept pharmacy residents as well as Doctorate of Pharmacy Candidates.
• Actively participate in the development, implementation, and monitoring of standards of care for clinical activities of pharmacists.
• Participate in departmental approved research.
• Document clinical activities according to departmental specifications. Serve on appropriate hospital committees or task forces as assigned. Keep abreast of current literature and developments in pharmacy.
• Develop and maintain an active clinical practice in order to maintain clinical expertise in the monitoring of patient drug therapies.
• Participates in the Clinical Pharmacy on-call schedule.
• Review medication orders and enter onto the patient's pharmacy profile. Evaluate the appropriateness of therapy and screens for drug-drug interactions, drug allergy interactions, therapeutic duplication, age appropriate dosing, or other contraindications.
• Dispense medications with appropriate labeling. Compound sterile and non-sterile products when requested.
• Respond to and assist during cardiac arrests and other medical emergencies. Knowledge, Skills & Abilities Patient Group Knowledge (Only applies to positions with direct patient contact) The employee must possess/obtain (by the end of the orientation period) and demonstrate the knowledge and skills necessary to provide developmentally appropriate assessment, treatment or care as defined by the department's identified patient ages. Specifically the employee must be able to demonstrate competency in: 1) ability to obtain and interpret information in terms of patient needs; 2) knowledge of growth and development; and 3) understanding of the range of treatment needed by the patients. Competency Statement Must demonstrate competency through an initial orientation and ongoing competency validation to independently perform tasks and additional duties as specified in the job description and the unit/department specific competency checklist. 2. Demonstrate positive customer service and co-worker relations.
3. Comply with the company's attendance policy.
4. Participate in the continuous, quality improvement activities of the department and institution.
5. Perform work in a cost effective manner.
6. Perform work in accordance with all departmental pay practices and scheduling policies, including but not limited to, overtime, various shift work, and on-call situations.
7. Perform work in alignment with the overall mission and strategic plan of the organization.
8. Follow organizational and departmental policies and procedures, as applicable.
9. Perform related duties as assigned. Education
• Bachelor's Degree (Required) Experience: 2 year residency required for ID, Critical Care, Oncology, and Peds positions OR 1 year residency required for Internal Medicine and Ambulatory Care positionSubstitution: In absence of required residency training, may substitute equivalent experience as follows:Eligible to serve as preceptor for applicable PGY1 and PGY2 residency programs and eligible to sit for BPS specialty certification in practice area when one is established or eligible to sit for Pharmacotherapy specialty certification for practice areas without an established specialty credential
