Registered Nurse Case Manager

As an RN Case Manager with Mercy, you will coordinate patient care across Watonga, Kingfisher, and Guthrie, ensuring safe, efficient transitions through the continuum. You’ll complete comprehensive assessments, develop individualized care plans, and collaborate with physicians, therapists, social workers, and community partners to optimize outcomes and reduce readmissions. Responsibilities include patient and family education, resource referral, utilization review, and thorough documentation in the EMR.
