Part Time / Weekend Physical Therapist Case Manager: Home Health

Full Time Physical Therapist Case Manager – Antioch Area We are looking for a skilled Physical Therapist to evaluate and address the needs of individuals in their homes, helping them live safely and productively. The role includes clinical assessment, treatment planning, case management, and coordination with interdisciplinary teams. What you’ll do Perform a home evaluation and develop a care plan, working with PTAs to follow the plan. Assist patients with mobility and movement to regain independence. Create detailed treatment and care plans for each patient. Communicate and coordinate with patients’ physicians to ensure quality care. Evaluate and report patients’ progress to MD. Qualifications Current unrestricted CA Physical Therapist License. Current BLS/CPR Certification. Valid CA Driver’s License. Knowledge of local community resources is required. Knowledge and understanding of individual development and human behavior as it relates to the effects of illness/injury and of the influence of culture on health care. Knowledge of state and federal home‑care regulatory guidelines preferred. Experience in home‑health care highly preferred. Experience with high‑acuity patients a plus. Experience working with an interdisciplinary team highly desired. Case‑management fundamentals highly desired. Self‑directed. Empathetic. Critical thinker. Outstanding interpersonal skills.
