PRN LPN

Job Summary:

TruHeart Home Health and Hospice is seeking a skilled and compassionate PRN Licensed Practical Nurse (LPN) to join our team in Tulsa, Oklahoma. As a PRN LPN, you will provide prescribed nursing care services to ill, injured, convalescent, and disabled patients in their homes, following a Plan of Care (POC) according to the patient's needs. This is a per diem position, with opportunities for flexible scheduling.

Responsibilities:

• Provide comprehensive health and psychological evaluations, monitoring of patient condition, health promotion, and prevention coordination of services, and direct nursing care.

• Demonstrate knowledge and competency in performing baseline nursing skills as outlined in agency policies and clinical policies.

• Implement nursing care to patients in the home setting, working under the direct supervision of an RN/MD.

• Administer medications and treatments according to Agency policy, accepted standards of care, and home care guidelines.

• Assist with rehabilitation according to the medical care plan, encouraging patients to help themselves using their own capabilities.

• Identify and determine priority of patient needs/problems, anticipate emergencies, and report abnormalities to the RN/MD.

Requirements:

• Current Oklahoma LPN license

• CPR certification

• Driver's license and auto insurance

• One or more years of experience in home health agency or hospital setting

• Competency assessments must be completed

• Graduate of an accredited school of nursing for LPN

• Ability to use critical thinking to resolve problems

• Strong communication and organizational skills

• Ability to work in a fast-paced environment with varying patient needs

• Ability to access patient homes, which may not be wheelchair accessible

• Hearing, eyesight, and physical dexterity sufficient to perform patient care
