Key Responsibilities\n

• Conduct comprehensive assessments of clients to identify their mental health needs and create personalized treatment plans.

• Coordinate with supervisors, family members, guardians, and other stakeholders to communicate clients' progress and treatment adjustments.

• Regularly provide a tentative schedule for appointments, home visits, and observations to monitor clients’ progress effectively.
\n
• Complete necessary home visits to perform in-depth observations and chart reviews, engage in staff interviews, and analyze data for continuous improvement.
\n
• Deliver tailored interventions and ensure the effective implementation of client program plans.
\n
• Participate in Interdisciplinary Team Meetings and other collaborative sessions to assess treatment developments.
\n
• Prepare detailed monthly and quarterly progress notes, documenting client advancements and treatment outcomes thoroughly.
\n
• Facilitate training modules for new staff and team members, focusing on the implementation of program plans and meticulous documentation procedures.
\n
• Provide ongoing follow-up interviews and home visits to support clients maintaining their progress and adapting to changing needs.
\n
• Maintain comprehensive client records to ensure data is easily accessible for all relevant parties.
\n
• Act as a liaison between clients, their families, and the mental health community, advocating for meaningful support and development goals.

• Assist in the creation and improvement of training materials aimed at enhancing program efficacy for clients.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• A demonstrated specialization in family therapy, couples therapy, substance issues, or trauma is highly preferred.
