Hospice Referral Growth Strategist

Gentiva is seeking a Hospice Care Consultant in Texas to grow our referral network and educate providers on hospice options. You will build and maintain relationships with hospitals, home health agencies, SNFs, and physician offices, while promoting patient-centered care.
