Job Description
LTC/SNF Registered Nurse (RN)

We are seeking dedicated and compassionate LTC/SNF Registered Nurses (RNs) to join our team. In this role, you will provide high-quality care to elderly patients with diverse medical conditions in a long-term care or skilled nursing facility.

Job Responsibilities

Deliver patient-centered nursing care in accordance with established protocols and standards.
Perform assessments and develop individualized care plans for residents.
Administer medications and treatments as prescribed by physicians.
Monitor vital signs and report changes in patients' conditions to the healthcare team.
Educate patients and their families about health conditions and care plans.
Collaborate effectively with multidisciplinary teams to ensure optimal patient outcomes.

Qualifications

Current and valid Registered Nurse (RN) license.
BLS Certification required.
Experience in long-term care or skilled nursing facilities preferred but not required.
Strong communication and interpersonal skills.
Ability to work collaboratively in a team-oriented environment.

Work Schedule

12-hour shifts; day shift.
Every other weekend and holiday availability is required.

Additional Information

Location-specific orientation provided, including 1 day of training followed by up to 3 days of hands-on experience with staff.
Common diagnoses include various conditions affecting elderly patients.
Patient-to-staff ratios vary; day shifts are typically scheduled at a ratio of 1:21, but may go up to 1:42.
