Job Summary:

Responsibilities:

• 
  Administer medications and treatments as prescribed by physicians.
  
  
• 
  Monitor residents' health status and document changes accurately.
  
  
• 
  Assist with activities of daily living to promote patient comfort.
  
  
• 
  Follow facility protocols and maintain patient safety at all times.
  
  
• 
  Communicate effectively with patients, families, and healthcare team members.
  
  

Qualifications:

• 
  Valid LVN or LPN license in the state of practice.
  
  
• 
  Proof of COVID-19 vaccination or valid exemption documentation.
  
  
• 
  Strong understanding of long-term care or SNF clinical procedures.
  
  
• 
  Ability to work independently and as part of a team.
  
  
• 
  Excellent communication and organizational skills.
