Case Manager, RN (Portland, OR / Hybrid / Per Diem)

We are looking for a Case Manager, RN for our Oregon team! Overview The Case Manager, RN is responsible for conducting initial in-home assessments, orientations for new patients and caregivers, and providing ongoing care management and support. This role combines clinical assessment, care coordination, patient and Personal Care Attendant (PCA) education, and quality assurance to ensure safe, effective, and person-centered care. The Case Manager, RN collaborates closely with patients, PCAs, and interdisciplinary care teams to assess needs, develop individualized plans of care, and facilitate the seamless initiation and delivery of services. Through regularly scheduled home visits, the RN evaluates patient and caregiver well-being, monitors the quality and effectiveness of care, and identifies opportunities to enhance health outcomes, safety, and regulatory compliance. The Case Manager, RN proactively communicates clinical concerns, changes in condition, and potential barriers to care with the appropriate team members to ensure timely intervention, effective care coordination, and optimal patient outcomes. Responsibilities Conduct initial in-person clinical assessments and coordinate service activation, ensuring that all required forms and standard procedures are followed. Develop, implement, manage patient activity plan(s). Make revisions and document changes as needed. Complete all client documentation, including written records, reports, and notes in accordance with agency policy and procedure following client visits or contacts in an accurate, timely, complete, concise and legible manner. Actively coordinate the care of the client through communication and written documentation to the physician and other disciplines involved in the client’s care and community resources. Provide ongoing coaching to drive effective utilization of established best practices, encourage PCA education, and connect families to appropriate resources. Review caregiver notes daily and appropriately address changes in the consumer’s condition or environment. Ensure continuous compliance with all regulatory standards as well as company policies and procedures. Perform other job-related duties as may be required or directed by the supervisor. Education and Experience Diploma from an accredited nursing program. New graduates welcome! Is a Plus Home care case management Care coordination experience Bilingual Qualifications Valid and Active Registered Nursing License issued by the Oregon Board of Nursing Ability to articulate cases through effective summaries. Ability and flexibility to travel (usually by car) within assigned geographic area Valid driver’s license and car insurance required. Proficient in common software and web-based applications (e.g. Case Management Systems, MS Office) with the ability to teach others. This provides our employees with the opportunity to professionally grow and develop within a role. The base pay range for this per diem role is between $45.00 and $50.00 per hour at the commencement of your first year of employment. Compensation decisions are dependent upon a variety of factors which may include, but are not limited to: skill set, years of relevant experience, education, location, and licensure/certifications.
