The LCSW will serve as part of the care team, collaborating, consulting, and referring, to coordinate all treatment needs.Description of Primary Responsibilities1) Provide necessary behavioral health services to patients identified by Palms Medical Group Providers to include, but not limited to, emergency assessment, care coordination, brief psychotherapy, and recommendation for higher level of care; in accordance with the policies and procedures of PMG.2) Promote health maintenance and prevention of disease.3) Promote mutual, beneficial professional, relationships between the corporation and the surrounding health service providers and institutes.4) Advise the appropriate member of management as defined on the institution's organizational chart of any problems perceived with PMG operations.5) Maintain the established system of medical records, in keeping with the current PMG policies and procedures.6) Be available after hours to treat or triage behavioral health emergencies as determined necessary.7) Share in providing coverage for extended office hours of PMG.8) Share in providing coverage for any other extended service approved by the Board of Directors (e.g., 24-hour coverage) as instructed.9) Agree to carry out all required responsibilities of the corporation relative to patient charges, third party reimbursement, ordering necessary therapy supplies and assessments, etc.10) Participate in community service and health promotion programs.11) Be responsible to participate in QA/QI activities as necessary.12) Be responsible for ongoing staff and provider education, peer reviews, staff competencies, and updates to committees on Behavioral Health Integration Project.13) Maintain appropriate certifications and undergo Credentialing, utilization, and meet requirements of peer reviews.14) Assume the supervision of any staff as requested by Chief Medical Officer in accordance with Florida statutory requirements.15) Perform other related duties as requested by the Chief Medical Officer.16) Participate in the review of all medical policies, procedures, and protocols.Description of Primary AttributesGeneral Development:1) Must be organized, a self-starter and detailed oriented2) Job duties require the ability to work independently and as part of a team3) Expected to travel and cover other sites during provider absences4) Expected to coach, mentor and train clinical staffProfessional & Technical Knowledge:1) Follow Safety and OSHA requirements2) Maintain appropriate certifications3) Undergo credentialing, utilization and peer reviews4) Employee will be expected to navigate and operate Microsoft Office suite products, including Word and ExcelLicenses & Certifications:1) Master's degree in social work from a Council on Social Work Education (CSWE) accredited school of social work2) Current and Valid LCSW license in the State of Florida3) Current BLS (basic life support) CertificationCommunications Skills:1) Effectively and tactfully communicates complex and/or technical information to co-workers, patients and/or vendorsPhysical/Mental/Emotional Demands:Standing for long periods of timeSitting for long periods of timeViewing a computer monitor for long periods of timeBendingStretching / ReachingWalking short distancesLifting up to 50 poundsOperating office equipment (computer, fax machines, telephones and copy machines)Reading forms / Instructions / Patients ChartsCommunicating well to people of various ages, educational levels, cultural backgrounds in person or by telephoneExposure to potentially violent / irate patientsHealth / Safety Consideration of PositionExposure to chemical infectious / contagious illnessExposure to chemical and inhalation of antibiotics during reconstitutionExposure to X-Ray radiationExposure to a variety of scents and odorsMust utilize universal precaution in clinical or exposure situations as prescribed by federal state, and local guidelines and /or laws
