Essential Functions: · Interview patients to obtain accurate, thorough health histories. · Performs physical examinations and provides preventive health services. · Orders, interprets and evaluates diagnostic tests to identify and assess patients\' clinical problems and health care needs. · Formulates comprehensive health care plan based on findings. · Prescribes or recommends medications or other forms of treatment. · Refers and consults with other professionals as needed. · Educates patients and family members about health care issues. · Ensures appropriate patient follow-up and reevaluation occur. · Actively participates in the integrated health care team, working conjunctively with the School Based Health Clinic, Care Team, and dental and behavioral health departments to ensure seamless, multidisciplinary patient care. Additional Requirements: · Grants are vital to ECH revenues and directly or indirectly support all positions. This role may have responsibilities that are closely associated with grant deliverables. Grants and grant responsibilities may change over Time. · Complete all required training as designated by HR and the position supervisor. · Adhere to all policies within the organization and those outlined within the ECH Employee Handbook. · Comply with all ECH Vaccine Mandates. · Willingness to travel and work at other ECH locations as determined by the supervisor. · Comply with all applicable Credentialing and Privileging requirements in a timely manner. · Embrace the ECH Mission and Vision and agree to work within the ECH Guiding Principles and the principles set by the position supervisor. · Complete additional duties as assigned by your supervisor. · Must participate in quality assurance/ improvement activities when applicable. · It may be scheduled for Saturday clinics and telehealth hours. Education/Experience: Must Haves to be Successful: · Master\'s Degree or Doctorate in Nursing. · At least two years experience in a family medicine or community health center setting and DEA preferred, · Interpersonal skills and sound judgment are necessary to communicate with a wide variety of persons, including the indigent, and to work with the Clinic staff in providing the clinic\'s medical services. · Exhibit cultural competence and exceptional teamwork skills. · Ability to read, write, and speak the English language. · Ability to interpret various instructions and deal with multiple linguistic and cultural variables. · Ability to relate cooperatively and constructively with patients, co-workers, administration, physicians and providers, community agencies, referral sources, regulators, and other health team members. · Ability to enlist the cooperation of others. · High tolerance for stress. · Excellent organizational and leadership abilities. · Outstanding communication and people skills. · Knowledge of the industry\'s legal rules and guidelines. Nice to Have: · Bilingual speakers are highly preferred. The work environment in a healthcare setting can be fast-paced and involves communication with patients and medical personnel.
