Home Health Physical Therapist – Pierce County Position Overview: Careage Home Health is hiring a Home Health Physical Therapist to provide one‑on‑one care to patients in their homes throughout Pierce County. This is a full‑time role traveling Monday through Friday, with an hourly wage of $50–$60 DOE. Benefits: Competitive hourly rate, comprehensive benefits, and travel time paid. Responsibilities Travel within Pierce/Thurston County to provide intermittent home‑based care. Administer physical therapy in accordance with physician orders and the plan of care under the supervision of the Director of Clinical Services. Collaborate with home‑care staff in patient care planning. Direct and supervise Physical Therapy Assistants and Home Health Aides as assigned. Take initial patient history and perform initial evaluations. Consult with physicians regarding changes in treatment. Instruct patients and families in home programs and activities of daily living. Participate in Quality Assessment and Performance Improvement activities as assigned. Complete and submit OASIS assessments, reassessments, transfers, resumptions, discharges, and significant changes in condition. Use ICD‑10 codes appropriately. Requirements Graduate from an APTA‑approved physical therapy curriculum. Licensed PT in the state of Washington. Minimum 2 years of physical therapy experience (home‑health experience preferred). Detail‑oriented, self‑motivated, and capable of efficient time management. Critical thinker able to work collaboratively with a team. Valid Washington state driver’s license. #J-18808-Ljbffr
