Seeking a dedicated and compassionate Physical Therapist to join a growing home health team. This role is ideal for clinicians passionate about patient-centered care who enjoy working with diverse populations in a flexible and supportive environment.

Responsibilities:

• Perform comprehensive physical therapy evaluations and assessments
• Develop and implement individualized treatment plans based on patient goals
• Deliver hands-on therapy using evidence-based techniques
• Educate patients and caregivers on exercises, safety, and recovery strategies
• Collaborate with interdisciplinary healthcare professionals to ensure coordinated care
• Maintain accurate and timely documentation using EMR systems
• Participate in discharge planning and continuity of care

Patient Populations:

• Adults and Pediatrics
• Geriatrics
• Orthopedics
• Home and community-based care

Qualifications:

• Licensed Physical Therapist or eligible for licensure
• Strong knowledge of anatomy, physiology, and rehabilitation principles
• Experience in outpatient, inpatient, or home health settings preferred
• Ability to work independently in a home health environment
• Excellent communication and patient education skills

Position Details:

• Caseload of 3-5 patients per day
• Flexible scheduling including day, night, and on-call shifts
• Weekend availability optional and preferred
• In-person, home-based care setting
