About the Role:

Join By Your Side Hospice Care LLC as a Hospice RN Case Manager, where you will provide compassionate care to patients and their families during life's most challenging moments. In this role, you will be an essential part of a dedicated team committed to enhancing the quality of life for our patients in South Eastern, Wisconsin.

Responsibilities:

Conduct comprehensive patient assessments and develop individualized care plans.

Coordinate and manage patient care, ensuring adherence to hospice policies and procedures.

Provide direct nursing care, including symptom management and pain control.

Educate patients and families on end-of-life care and available resources.

Collaborate with interdisciplinary teams to optimize patient outcomes.

Document patient progress and maintain accurate medical records.

Participate in on-call rotations to provide after-hours support.

Advocate for patients' needs and preferences in care decisions.

Requirements:

Current RN license in the state of Wisconsin.

Minimum of 2 years of nursing experience, preferably in hospice or palliative care.

Strong assessment, communication, and interpersonal skills.

Ability to work independently and as part of a collaborative team.

Compassionate demeanor with a focus on patient-centered care.

Experience with electronic medical records (EMR) systems.

Certification in hospice and palliative nursing (CHPN) is a plus.

Willingness to travel within the service area as needed.
