Rehab Patient Care Coordinator - (PT/OT/SLP)

Are you a licensed Physical Therapist, Occupational Therapist, or Speech-Language Pathologist looking to expand your impact beyond direct patient care? Join our growing home health team as a Patient Care Coordinator!

In this role, you'll help coordinate multidisciplinary patient care, collaborate with clinicians and office staff, ensure regulatory compliance, and support exceptional patient outcomes.

What You'll Do:
• Coordinate and manage patient care services
• Serve as a liaison between patients, families, providers, and caregivers
• Manage referrals, authorizations, scheduling, and care plan coordination
• Participate in interdisciplinary case conferences
• Monitor patient progress and ensure continuity of care
• Support quality assurance, chart audits, and regulatory compliance
• Collaborate with clinical and administrative teams to improve patient satisfaction and outcomes

Qualifications:
✔ Active California PT, OT, or SLP License
✔ Current BLS Certification
✔ Home Health experience preferred
✔ Knowledge of Medicare regulations and OASIS preferred
✔ Strong organizational, communication, and computer skills
✔ Leadership, care coordination, or management experience is a plus
