RN Case Manager

Job Description

Day-to-Day:

This is a fully on-site, patient-facing role where RN case managers are assigned to specific units-including the Weaver Tower (Neuro/Stroke and Oncology in partnership with MD Anderson), the Heart Hospital (Cardiovascular and Open Heart), or the Main Tower (Med-Surg, Orthopedics, and Bariatrics)-and work collaboratively within an interdisciplinary team of physicians, bedside RNs, social workers, and care coordination assistants, participating in or leading daily progression-of-care rounds, with no people management responsibilities. Duties include high risk screening, assessment, discharge planning, and coordination of post-acute care services.

Skills and Requirements

Must-Haves:

RN with Case Management experience (2+ years ideal)

Strong payer / insurance knowledge

Transitions of care experience

Comfortable in fast-paced, high-throughput environment

Confident communicator with physicians

Experience leading or contributing to interdisciplinary rounds Soft Skills

Problem solver / critical thinker

Team-oriented

Confident but collaborative

Adaptable and system-quick learner
