University Home Care Inc is seeking a compassionate and skilled Home Care Physical Therapist to join our team in Livonia, MI. In this role, you will provide high-quality physical therapy services to patients in their homes, helping them improve mobility, manage pain, and enhance their overall quality of life.
Responsibilities- Conduct in-home physical therapy evaluations and assessments
- Develop and implement individualized treatment plans based on patient needs
- Provide therapeutic interventions to improve patient mobility and function
- Monitor patient progress and adjust treatment plans accordingly
- Educate patients and their families on exercises and care techniques
- Maintain accurate and timely documentation of patient care
- Collaborate with healthcare providers to ensure continuity of care
- Valid Physical Therapist license in the state of Michigan
- Experience in home health or outpatient physical therapy preferred
- Strong interpersonal and communication skills
- Ability to work independently and manage a flexible schedule
- Reliable transportation for travel to patient homes in the Southeast MI Region
- Commitment to providing compassionate patient-centered care
- Competitive hourly rate: $70.00 - $80.00 per visit, paid bi-weekly
