Role Description The Registered Nurse will provide direct patient care in clients’ homes, including comprehensive assessments, development and implementation of individualized care plans, and ongoing monitoring of patient status. Day-to-day responsibilities include wound care, IV line management, chronic disease management, medication administration, and collaboration with physical, occupational, and speech therapists to coordinate interdisciplinary care. The RN will educate patients and families on disease processes, treatments, medications, and home safety, while documenting all care in accordance with regulatory and agency standards. The role also involves communicating with physicians, case managers, and other care team members to ensure continuity of care and timely updates. This is a Part-time/PRN, on-the road role serving clients in the Greater Houston area. Qualifications Active Registered Nurse (RN) license in the state of Texas and graduation from an accredited nursing program. Experience in home health or acute care settings, with skills in wound care, IV line management, and chronic disease management. Competence in medication administration and monitoring for changes in patient condition. Ability to perform comprehensive patient assessments, develop care plans, and coordinate with other providers. Strong communication, documentation, and patient education skills, with a focus on safety, dignity, and culturally sensitive care. Reliable transportation, valid driver’s license, and ability to travel within Greater Houston to provide on-site home visits.
