Position Title: Licensed Practical Nurse (LPN) – Long Term Care (Days)
Location: Meeker, Colorado
Job Type: Contract
- 3 x 12-hour shifts per week
- Day Shift
- 36 guaranteed hours per week
- Holiday coverage required
"Navitas Healthcare, LLC" is an experienced Licensed Practical Nurse (LPN) to provide high-quality care in a Long Term Care setting. The role requires strong clinical judgment, independence, and prior experience in skilled nursing or LTC environments.
Key Responsibilities- Provide direct patient care including bathing, feeding, and toileting
- Administer medications and treatments as prescribed
- Monitor patient conditions and report changes promptly
- Maintain accurate and timely documentation
- Assist with care planning and MDS documentation (preferred)
- Ensure compliance with safety and infection control standards
- Collaborate with interdisciplinary healthcare teams
- Active, unrestricted Colorado LPN License
- Minimum 3 years of Long Term Care (LTC) experience
- Current BLS/BCLS certification
- Experience with MDS documentation
- Experience using PointClickCare EMR
- Strong clinical judgment and patient care skills
- Ability to work independently in a long-term care environment
- Strong documentation and communication skills
- Team collaboration and resident-focused care approach
