Responsibilities:

Conduct patient evaluations, physical exams, and health screenings in a home setting Diagnose and manage acute and chronic conditions Prescribe medications and develop personalized care plans Collaborate with a multidisciplinary team to ensure the best patient outcomes Provide preventative care and patient education

Requirements:

Active NP license in New York Board-Certified Family Nurse Practitioner (FNP) Experience in home health, primary care, or geriatrics preferred Strong communication and patient care skills
