Ardor Health Solutions is seeking an experienced Home Health Physical Therapist for a 13-week assignment in Roseburg, Oregon. This opportunity is ideal for a therapist who enjoys autonomy, flexible scheduling, and building meaningful relationships with patients in the home setting.

Assignment Details

Requirements

• Active Oregon PT License
• Minimum 1 year of Home Health experience required
• Home Care Home Base experience required
• OASIS experience required
• Current BLS Certification
• Valid driver's license and reliable transportation
• Ability to work independently and manage a daily patient schedule
