RN Case Manager Home Health Full Time, Days

Responsibilities
- Develop and collaborate on care plans with the patient's physician and caregivers upon admission.
- Periodically review and recertify patient care plans as necessary.
- Coordinate and communicate patient care plans through interdisciplinary communication and case conferences.
- Notify other disciplines of discharges promptly.
- Perform ongoing assessments of the patient's clinical, functional, and emotional status to monitor progress and identify issues.
- Communicate significant changes in the patient's condition to their physician and VNA colleagues.
- Document evaluations, treatment goals, and objectives effectively.
- Educate patients and caregivers on self-care techniques and provide health counseling.
- Administer medication, dietary, and other instructions as prescribed by the physician.
Required Experience / Certifications / Licensure
- Must be a licensed Registered Nurse in Vermont and New Hampshire or possess an unencumbered compact license.
- Graduate of an accredited school of nursing.
- A minimum of 2 years of experience in a Home Health setting.

This is a full-time, day shift position.
Location
Multiple locations available including Bradford, VT; Randolph, VT; Springfield, VT; White River Junction, VT; and Lebanon, NH.
