Key responsibilities of the LPN/LVN Long Term Care

Provide direct patient care and assist with daily living activities

Administer medications and treatments as prescribed

Monitor patient progress and report changes to the healthcare team

Maintain patient records and documentation accurately

Requirements of the LPN/LVN Long Term Care

Current LPN/LVN license in Pennsylvania

Experience in Long Term Care settings preferred

Basic Life Support (BLS) certification

Strong communication and interpersonal skills
