Location: Glendale, CA (office-location)

Position: Clinical Liaison VIPC, Hospice

Position Type: Part-Time

Remote/Virtual Position: No

Coverage Area: Huntington Hospital / field-based compliance visits when there aren't Huntington Hospital patients

Salary: $55 - $60 / hour Schedule: Every Other Weekend shift (20 hours) 8am-6:30pm

Key Responsibilities:

• Build and maintain trusted relationships with physicians, hospital staff, and care teams through daily in?person collaboration and communication.
• Serve as the primary clinical resource for hospice education, eligibility, and appropriateness.
• Conduct patient assessments and coordinate the admission process.
• Partner with Admissions, Clinical, and Care Teams for smooth transitions.
• Advocate for patients and families during goals?of?care conversations.
• Participate in interdisciplinary planning, training, and outreach.
• Uphold ethics, quality, and continuous learning.
• Meet expectations for response times, referral coordination, and communication.

If you meet these qualifications, we want to meet you!

• One year of RN experience with knowledge of home health regulations, Medicare requirements, and care?plan development
• Medical sales or clinical outreach experience preferred
• Strong communication and relationship?building skills
• Ability to work independently in a hospital environment
• Passion for serving with dignity and empathy
• Active RN license
• Valid driver's license and insured vehicle
