A Client Care Navigator (CCN) is a very important role within the Isaiah House organization. Navigators represent the treatment programs offered by Isaiah House to front-line referral agencies throughout the state of Kentucky. Employees receive benefits that include: Medical, dental, and vision insurance Basic life insurance 401(k) Option Paid holidays Additional Information: Schedule: Full-time, Monday - Friday, 8:00 AM - 4:30 PM Travel Requirements: 100% Travel: Rockcastle, Pulaski, Russell, Cumberland, Clinton, Wayne and McCreary County. Key Job Responsibilities: Build and maintain strong relationships with Centers of Influence (COIs) within the assigned CCN region. Educate COIs on Isaiah House’s history, programs, services, and client outcomes. Field calls from potential clients exploring treatment options and communicate all new client referrals to the CCN Supervisor through Microsoft Teams. Manage travel schedules for assigned counties within the region. Maintain accurate records of COI activity, client referrals, and client admissions in Asana. Required Qualifications: High school diploma or GED preferred. Knowledge of addiction and substance use treatment preferred. Strong connections with community services, organizations, and public agencies preferred. Valid driver’s license required.
