Senior Community Care is seeking a Clinical Dietitian to provide medical nutrition therapy for seniors in our PACE program. In this interdisciplinary role, you will complete comprehensive nutrition assessments, develop and monitor individualized care plans, and collaborate closely with physicians, nurses, therapists, and social workers to support safe, independent living. You will educate participants and caregivers, document in the EHR, and uphold regulatory standards while contributing to our compassionate, mission-driven culture focused on dignity, respect, and holistic senior care.
