This position offers a 5x8.0 schedule with 60-minute evaluations and 30-minute follow-ups.

Job Description

The Outpatient Physical Therapist is responsible for evaluating all assigned patients and developing, implementing, and modifying treatment plans based on patient needs, goals, and physician direction. This role includes delegating and directing discipline-specific therapy services and ensuring the design of functional treatment programs that facilitate patient return to community activities. The therapist will also serve as an educator for patients, families, students, and coworkers.

Responsibilities

• Conduct comprehensive evaluations of assigned patients.
• Develop appropriate treatment plans and goals focused on patient discharge.
• Document medical records in a clear, concise, accurate, and timely manner.
• Participate in the student program, providing supervision and evaluation.
• Maintain professional licensure and complete necessary continuing education.
• Promote patient and employee safety through effective practices.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• Minimum of 6 months to 1 year of outpatient clinical experience.
• Current CPR certification.
• Must adhere to the discipline-specific State Practice Act and maintain licensure in good standing.
