Job Summary:

Holly Hill Healthcare Center, a 75-bed skilled nursing, long-term care, rehabilitation, and memory care-focused facility located in Towson, Maryland, is seeking compassionate, dependable, and service-oriented Licensed Practical Nurses (LPNs) to join our dedicated clinical team. As an LPN at Holly Hill, you will play a vital role in delivering high-quality, resident-centered care while supporting the health, safety, dignity, comfort, and quality of life of the residents we serve.

The Licensed Practical Nurse provides direct nursing care, administers medications and treatments, monitors resident conditions, and collaborates with an interdisciplinary team to support positive clinical outcomes. Responsibilities:

• Provide direct nursing care to residents in accordance with physician orders, individualized care plans, facility policies, and applicable nursing standards

• Administer medications, treatments, and nursing procedures safely and accurately

• Observe, assess, and document resident conditions while promptly reporting changes to appropriate clinical leadership

• Maintain accurate and timely clinical documentation and resident records

• Collaborate with physicians, Registered Nurses, Certified Nursing Assistants (CNAs/GNAs), rehabilitation professionals, and interdisciplinary team members

• Participate in resident admissions, discharges, transfers, care planning, and clinical assessments as assigned

• Support and guide nursing assistants and other assigned team members to promote quality resident care

• Follow infection prevention, safety, quality assurance, and regulatory compliance standards

• Promote resident rights, dignity, privacy, comfort, independence, and overall quality of life
  

Minimum Qualifications:

• Current, active Maryland Licensed Practical Nurse (LPN) license in good standing

• Graduate of an accredited practical nursing program

• Current CPR certification or ability to obtain upon hire

• Strong clinical assessment, communication, organization, and documentation skills

• Ability to work effectively in a fast-paced skilled nursing environment

• Long-term care, rehabilitation, skilled nursing, or memory care experience preferred
