Registered Nurse Case Manager

Visiting Nurse Association of Wisconsin, Inc., affiliated with Aurora Health Care, is seeking a Registered Nurse Case Manager to provide compassionate, patient-centered care in patients’ homes and communities across Wisconsin. Assess patients, develop and update individualized care plans, coordinate services with providers and interdisciplinary teams, and monitor progress through recovery and chronic disease management. Educate patients and families, document care accurately, and support hospice and end-of-life needs as appropriate. Collaborate respectfully with diverse communities while upholding clinical excellence and integrity. This role offers meaningful impact, professional development, and the support of a larger health system. A current Wisconsin RN license and strong assessment, communication, and care-coordination skills are required; relevant clinical experience is preferred. Home visits and local travel are expected.
