Physical Therapist Assistant, Home Care, Bronx

VNS Health, a leading provider of home and community-based healthcare services in New York City, is seeking a dedicated Physical Therapist Assistant for home care in the Bronx. In this role, you will work under the supervision of a licensed Physical Therapist to deliver compassionate, evidence-based rehabilitation services to patients in their homes. You will carry out established care plans, assist with therapeutic exercises, functional mobility training, gait and balance activities, and use of assistive devices to help patients regain independence and improve their quality of life.

As a Physical Therapist Assistant with VNS Health, you will visit patients across diverse Bronx neighborhoods, carefully observing and documenting their progress, reporting changes in condition, and collaborating closely with the supervising Physical Therapist, nurses, and other members of the interdisciplinary care team. You will provide clear instruction and education to patients and caregivers on home exercise programs, safety techniques, and strategies for maintaining function in the home environment. Your work will emphasize patient-centered care, cultural sensitivity, and respect for the unique needs of each individual and family.

You will use strong organizational and time-management skills to manage a home-care caseload, complete timely and accurate electronic documentation, and coordinate visit schedules.
