What This Role Offers • Monday Friday daytime schedule • Minimal call requirements • No overnight hospital rounding • A consistent patient panel within LTC communities • Strong interdisciplinary collaboration • Administrative and documentation support While some visits are brief and focused, the overall model prioritizes continuity of care and long-term patient relationships particularly for medically complex seniors. Clinical Responsibilities • Conduct admissions and new patient evaluations • Manage chronic and acute medical conditions in the LTC setting • Perform routine follow-ups and regulatory visits • Collaborate with NPs, nursing staff, therapists, and facility leadership • Communicate with families and caregivers • Document encounters using a company-issued mobile device The focus is on quality, coordination, and patient-centered care within a structured post-acute environment. Schedule • Monday Friday • Daytime hours • Minimal on-call rotation • No regular nights or weekend rounding Predictable schedule. Protected personal time.
