The Licensed Practical Nurse (LPN) provides direct patient care in a community health setting under the supervision of a Registered Nurse (RN) or licensed provider. The LPN collaborates with the interdisciplinary care team to promote safe, efficient, and patient-centered care in accordance with state scope of practice and organizational policies.

Shift: This is a Monday-Friday position, NO weekends, NO on-call responsibilities!

Pay Rate: $24-30/hour, commensurate upon experience. SIGNING BONUS: Up to $1500!

Duties & Responsibilities:

• The LPN provides direct patient care under the supervision of an RN or licensed provider in accordance with state scope of practice.
• Perform supervised, focused assessments, collect patient data, and document accurately in the electronic health record.
• Administer medications and immunizations, including intramuscular (IM), subcutaneous (SQ), and oral consistent with training and policy.
• The LPN Prepares and assists providers with examinations and procedures, including GYN exams and minor in-office procedures.
• Perform point-of-care testing, specimen collection, EKGs, and related clinical tasks.
• Provide support in pediatric, adult, and women's health visits.
• The LPN assists with substance use disorder services, including urine toxicology collection, and reinforcement of treatment plans as directed by licensed providers.
• Reinforce patient education based on previous documentation from chronic disease case management RNs' care plans, including medications, lifestyle modifications, and preventive care instructions.
• Complete clinical paperwork, prior authorizations, referrals, disability/FMLA forms, and other required documentation in collaboration with providers and RNs.
• Conduct follow-up calls and outreach to patients regarding appointments, and care gaps.
• Conduct follow-up calls and outreach to patients regarding minimally abnormal lab results based on documented education provided by RNs or licensed providers.
• Inform patient of normal lab results tasked by licensed providers.
• Monitor and report changes in patient condition to the supervising RN or licensed provider.
• Maintain exam room readiness, infection control standards, and clinical supply inventory.
• Participate in quality improvement initiatives and support population health efforts.
• Collaborate with interdisciplinary team members to ensure coordinated, patient-centered care.
• Assist Medical Assistants with rooming patients when short staffed.
• Appropriately answer patient calls from the call center, provide basic information within scope, and ensure a warm hand-off to RNs or providers for issues beyond LPN scope.

• Current Massachusetts LPN nursing license.
• Current CPR certification.
• Bilingual preferred.
• Two years' experience in a hospital or community health center setting (new graduates are encouraged to apply).
