Schedule: Monday – Friday, 8:00 AM – 4:30 PM
Position: 748V BHS Hospital Based – Social Worker
Local Candidate Requirement: Candidates must reside more than 50 miles from the facility
Setting: Acute Hospital / Behavioral Health

Position Overview

We are seeking an experienced Social Worker for a 12-week contract assignment supporting a hospital-based Behavioral Health Services (BHS) team in Lawrenceville, GA.

The Social Worker will provide comprehensive psychosocial assessment, care coordination, discharge planning, counseling, and support services to patients and families in an acute hospital setting . The ideal candidate will have at least two years of specialty experience, strong clinical communication skills, and experience working with interdisciplinary healthcare teams.

Key Responsibilities

• Conduct psychosocial assessments of patients and identify social, emotional, behavioral, and environmental needs.
• Provide appropriate social work interventions and support to patients and families.
• Assist patients in navigating healthcare services, community resources, and support programs.
• Develop and coordinate individualized discharge plans in collaboration with the healthcare team.
• Coordinate transitions of care and facilitate appropriate referrals to community and post-acute services.
• Collaborate with physicians, nurses, behavioral health professionals, case management, and other interdisciplinary team members.
• Provide crisis intervention and support for patients experiencing behavioral health or psychosocial challenges.
• Advocate for patients and help address barriers to treatment, discharge, and continuity of care.
• Maintain accurate and timely documentation in accordance with hospital policies and applicable standards.
• Communicate effectively with patients, families, caregivers, and healthcare professionals.
• Assist with identifying appropriate community resources and services based on patient needs.
• Maintain patient confidentiality and comply with applicable healthcare privacy requirements.
• Participate in interdisciplinary meetings, care planning, and case reviews as required.
• Follow hospital policies, procedures, and clinical standards.

Required Qualifications

• Minimum 2 years of experience within the Social Work specialty .
• Acute hospital experience is required.
• Active Georgia LCSW (Licensed Clinical Social Worker) license required.
• AHA BLS certification required.
• Behavioral Health/hospital-based social work experience preferred.
• Strong assessment, communication, care coordination, and discharge planning skills.
• Ability to work effectively in a fast-paced acute-care environment.
• Strong documentation and organizational skills.
• Ability to collaborate effectively with multidisciplinary healthcare teams.
