Requirements
- Current state license as a Registered Nurse
- Homecare Homebase Software Experience
- OASIS Experience
- 1-2 years of experience as a Registered Nurse in a clinical care setting
- 2+ years of home health experience
- Proof of current CPR certification
- Current driver's license
- Reliable transportation and insurance

Roles & Responsibilities
- Assess home health and/or hospice patients in both stable and emergent situations to identify their physical, psychosocial, and environmental needs.
- Implement, develop, document, and evaluate the Plan of Care (POC) to assure quality and continuity of care.
- Provide guidance and education to patients and families regarding care, medications, and home safety with appropriate teaching methods.
- Supervise and provide clinical direction to healthcare aides to ensure quality service delivery.
- Monitor assigned cases to ensure compliance with third-party payer requirements and quality standards.
- Maintain effective communication with patients, families, and the interdisciplinary team to coordinate care.
