Nurse Practitioner - Primary Care & Senior Services (PRN Weekend)
This PRN weekend role provides primary care services through a combination of home visits and visits to assisted living and long-term care communities. Providers are scheduled based on weekend patient needs and work closely with the clinical and scheduling teams to ensure continuity of care.
Key Responsibilities
- Deliver comprehensive primary care during scheduled weekend shifts.
- Conduct new patient evaluations and follow-up visits.
- Assess, diagnose, and manage acute and chronic medical conditions.
- Develop individualized treatment plans and prescribe medications as appropriate.
- Order and interpret diagnostic testing.
- Coordinate care with specialists and other healthcare providers.
- Educate patients and caregivers on disease management and preventive care.
- Complete accurate EMR documentation within 24 hours of each encounter.
- Collaborate with the Medical Director and interdisciplinary care team to ensure quality outcomes.
Schedule
- PRN weekend coverage
- Flexible scheduling based on availability and patient volume
- Typical visit hours: 8:00 AM - 5:00 PM
- No on-call responsibilities
- Two-week orientation provided
- Credentialing may take up to 30 days
- Optional shadowing available
Service Area
- Richmond, VA (up to a 40-mile service radius)
Minimum Qualifications
- Active Virginia Nurse Practitioner license
- Current DEA registration
- Active CAQH profile
- At least one year of clinical experience in primary care, family medicine, senior care, assisted living, long-term care, or a related setting
- Experience with electronic medical record (EMR) systems
- Ability to work independently while collaborating with an interdisciplinary care team
- Reliable transportation for travel throughout the assigned service area
Preferred Qualifications
- Previous experience in home-based primary care
- Experience caring for geriatric or medically complex patients
- Background in urgent care, emergency medicine, hospital medicine, or post-acute care
- Strong communication and care coordination skills
