Occupational Therapist, Home Health

CenterWell Home Health is seeking a compassionate and skilled Occupational Therapist for in-home care to support patients in regaining independence and optimizing their quality of life where they feel most comfortable—at home. As an Occupational Therapist with CenterWell, you will provide individualized, one-on-one therapeutic interventions, evaluating functional abilities, developing patient-centered care plans, and delivering evidence-based treatments to help patients safely perform activities of daily living, manage chronic conditions, and reduce rehospitalizations. You will conduct comprehensive in-home assessments, identify functional and environmental barriers, recommend and train on adaptive equipment, and collaborate closely with nurses, physical therapists, physicians, social workers, and caregivers to ensure integrated, high-quality care. Documentation of assessments, progress notes, and care plans will be completed accurately and timely in accordance with regulatory and payer requirements. You will educate patients and families on home exercise programs, safety strategies, and energy conservation techniques, while reinforcing CenterWell’s focus on patient autonomy, safety, and measurable outcomes. In this role, you will manage a mobile caseload, plan efficient home visit schedules, and adapt interventions to diverse home environments and cultural backgrounds.
