As a Home Health Registered Nurse Care Manager, you will plan and direct home care services for patients in their homes, collaborating with a multidisciplinary team to ensure the highest quality of care. You'll manage a patient caseload, conduct assessments, and implement care plans while providing education and support to patients and their families.
