Targeted Case Manager - Medication Assisted Treatment

Job Summary Targeted Case Manager serves individuals with mental health and substance abuse disorders who require assistance to gain access to necessary care: medical, behavioral, social, and other services appropriate to their needs. Services include case management assessment, person-centered planning, referral and linkage, monitoring, and follow-up. The Targeted Case Manager will follow specific policies and procedures for the Family Safety Program and Family Intensive Treatment. Essential Duties and Responsibilities Provide case management functions according to CIHA policy and procedure and DMA Clinical Coverage Policy 8L: assessment, person-centered planning, referral, linkage, monitoring, and follow-up. Document all treatment and clinically significant events in the Electronic Health Record as prescribed by CIHA policies and quality assurance standards. Fulfill duties outlined in CIHA policy for adults, children and families, or the Buprenorphine Clinic as applicable. Gather and document necessary information across medical, psychosocial, financial, and other areas. Develop and implement person-centered treatment plans aimed at clinical outcomes. Provide timely information on client status, progress, and safety to the treatment team. Apply judgment in risk assessment and crisis intervention per CIHA policy. Collaborate with medical staff and other professionals to facilitate care, discharge, and follow-up. Offer consultation to patients and families on relevant matters. Develop and implement discharge plans and transfer to other behavioral health facilities as appropriate. Maintain privacy and confidentiality in patient records following relevant regulations. Attend clinical staffing meetings and contribute to program improvement. Provide consultation services to external organizations such as schools, courts, and community agencies. Represent CIHA during team and emergency meetings. Participate in community planning and program implementation. Collect and report data necessary for client care as directed by supervisor. Obtain continuing education through in-service training and approved seminars. Meet productivity standards for billable/reimbursable Targeted Case Management services as required. Utilize the CIHA electronic medical record and required software effectively. Support CIHA primary care nurse care managers as needed. Perform other duties as assigned. Education, Experience, Minimum Qualifications Valid license, provisional license, certificate, registration, or permit issued by the relevant governing board, or a full-time four-year experience in mental health, substance use, or related field for a non‑registered nurse with a state license. Master’s degree in a human service field with one year of full-time pre‑ or post‑graduate experience, or equivalent for substance abuse professionals. Bachelor’s degree in a human service field with two years of full‑time experience, or equivalent for substance abuse professionals. Bachelor’s degree in another field with four years of full‑time experience, or equivalent for substance abuse professionals. Job Knowledge Knowledge of the population served and Cherokee culture (preferred). Skilled in establishing rapport and applying assessment techniques. Ability to maintain effective working relationships across disciplines and community resources. Strong communication and planning skills. Knowledge of crisis intervention and complex casework techniques. Awareness of medical, behavioral, and psychosocial problems and appropriate linkages. Understanding of laws, regulations, and policies governing the program. Willingness to learn about Cherokee culture and population considerations. Complexity of Duties This position requires interpreting policy, laws, and regulations to make high‑stakes decisions, often dealing with highly agitated or emotionally distressed individuals. Supervision Received Works directly under the Family Safety Supervisor, Child Services Manager, and/or Behavioral Health Clinical Director with defined goals and policies. Operates independently within licensing regulations and is granted latitude for creative thinking. Responsibility for Accuracy Errors in documentation or interpretation can adversely affect client care and tribal resource allocation. Accuracy is critical per CIHA policy and procedure. Contact With Others Interacts with coworkers, hospital staff, patients, and other health professionals; may serve on committees and advisory councils. Confidential Data Maintains strict confidentiality of patient information in accordance with EBCI, CIHA, HIPAA, and other federal regulations. Emergency Privileges Allows authorized Medical Staff to perform necessary actions to save a patient’s life or prevent serious harm when no more qualified member is available. Mental, Visual, Physical Environment Job duties can be mentally and emotionally stressful. Physical demands include mobility, standing (under one third of time), walking, sitting, and occasional lifting of up to 30 lbs. Required visual acuity for close vision. Work Environment Primarily located at the PHHS building, with occasional community service, inpatient or outpatient clinical work, and possible travel, including infrequent overnight travel for meetings or training.
