Clinical Social Worker, Per Diem, Faulkner Hospital

Job Summary The Clinical Social Worker is responsible for assuming assigned patient caseloads on inpatient medicine and surgery, the Addiction Recovery Program, or the Emergency Department. The role provides bio-psychosocial assessments and interventions for patients and families impacted by acute medical needs and psychosocial barriers. The social worker identifies high-risk psychosocial factors, collaborates with interdisciplinary teams to address barriers to care, delivers therapeutic interventions, provides resources and recommendations, and aids in discharge planning. Knowledge of mandated reporting, cultural and age‑specific issues, homelessness, substance use disorders, psychiatric illness, and community resources is required. Leading groups for the inpatient addiction program is preferred. Responsibilities Assume assigned patient caseloads on inpatient medicine, surgery, Addiction Recovery Program, or Emergency Department. Provide bio‑psychosocial assessments for patients and families. Identify high‑risk psychosocial factors influencing patient health status. Offer evaluation, emotional supportive therapy, and other therapeutic modalities. Provide resources, recommendations, and team collaboration around discharge and community barriers. Maintain knowledge of mandated reporting and relevant community agencies. Lead groups for the inpatient addiction program (preferred). Qualifications Master’s Degree in Social Work from an accredited program. Current professional licensure in Massachusetts (LCSW required, LICSW preferred). Minimum of one year experience in a health‑care setting or related field. Graduate‑level internship experience in an appropriate setting. Knowledge of specific medical/psychiatric illnesses, procedures, and treatments. Excellent interpersonal and negotiation skills for team collaboration. Rapid clinical psychosocial assessment and intervention skills. Advanced crisis intervention skills. Ability to work effectively as a member of a multi‑disciplinary team. Knowledge of community agencies/resources. Advocacy and negotiation skills for patient systems. Tolerance for ambiguity. Cultural sensitivity. Adherence to all C.A.R.E. Standards.
