Travel Physical Therapist — Home Health — Mayfield, KY

Join a home health team in Mayfield, KY as a traveling Physical Therapist. Provide evaluations and skilled therapy services to patients in their homes across Graves and Calloway counties. Minimum Qualifications

• Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Kentucky physical therapy license required to start the assignment.
• Basic Life Support (BLS/CPR) certification required.
• Ability to perform patient evaluations for therapy services in home health settings.
• Willingness and ability to travel within coverage areas including Graves County and Calloway County.
• Minimum education requirement: Bachelor’s degree.

Preferred Qualifications

• Home health experience preferred.

Assignment Details

• Shift hours: 2000–0400.
• On-call/weekend requirement: No.
• Mileage reimbursed between visits, from first patient to last patient.
• Coverage areas include Graves County and Calloway County.
