Individual Support Specialist/Direct Support Specialist

Job Description

We are seeking Individual Support Specialist/Direct Support Specialist to join our team! You will be responsible for helping individual with intellectual disabilities navigate services while in their home or in the community. Hiring in Charlotte, Concord, Huntersville and surrounding areas.

Responsibilities:

Provide one-on-one support to individuals with intellectual and developmental disabilities
Assist individuals with daily living activities and independent living skills
Support individuals in community integration and participation in recreational activities
Assist with transportation to appointments, community outings, work, or activities as needed
Help individuals build social, communication, and coping skills
Implement individualized service plans and document progress toward goals
Encourage independence while providing supervision and support as needed
Maintain professional boundaries and ensure confidentiality at all times
Monitor health, safety, and overall well-being of individuals served
Complete required documentation, service notes, and incident reports accurately and on time
Collaborate with families, supervisors, and other providers to ensure quality care

Qualifications:

Previous experience in mental health /IDD a plus
HS Diploma or GED
Valid Driver's License
Personal Vehicle with current registration and insurance
Ability to complete required agency trainings and/or certifications
