Role: Full-Time Physical Therapist specializing in Home Health care, responsible for assessing, evaluating, and developing treatment plans to improve patient well-being. Key Responsibilities: Evaluate patients' functional status, mobility, and medical conditions. Assist in developing and implementing personalized therapy plans in collaboration with physicians. Provide therapeutic interventions to relieve pain, restore function, and maintain maximum mobility. Monitor and report patient responses and progress to healthcare team members. Educate patients, families, and caregivers about treatment plans and functional goals. Complete patient evaluations, progress notes, and develop care plans within state guidelines. Adapt to diverse home environments and ensure quality care regardless of client background. Qualifications & Skills: Current Physical Therapy licensure in the state of practice. Valid driver’s license, dependable transportation, or access to public transit. Preferred: CPR certification or ability to obtain within 90 days.
