Shift:
DAYS 8:30A-5P M-F   Work Experience:

• Assess the psychosocial needs of individuals, partners and families
• Conduct social work assessment and provide on-going social work services
• Prepare comprehensive care plan and evaluate progress with clients
• Provide counseling for individuals, families and groups and refer patients for psychiatric evaluation and treatment as needed
• Provide HIV counseling and testing

Education:

· Bachelor’s degree in health-related field (i.e. community health, Social Work

License: MSW LMSW   Certifications:
BLS/CPR Certification.
