Field RN Case Manager — Home Health on the Road

A mid-size home health care company is seeking a Home Health RN Case Manager for PRN or Full Time positions in the Grapevine area. Candidates must be licensed in Texas and have previous home health care experience. The role includes conducting assessments, managing care plans, and providing skilled nursing services. Ideal candidates will have strong medication management skills and the ability to educate patients and families.
