Primary Care Provider (PACT Nurse Practitioner)

Summary The Primary Care Provider (Nurse Practitioner/NP) provides comprehensive clinical care - consultation - and patient management in support of the VA mission. They practice to the full extent of their training and certification - follow defined clinical privileges - and hold prescriptive authority for their assigned patient panel or program. Responsibilities Start your career here The Primary Care Provider (NP) is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients Duties of the position include are but not limited to: Manage a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice Develop and implement individualized plans to achieve optimal population health and disease management Demonstrate leadership and work collaboratively with Primary Care Team and other interdisciplinary team members Order and interpret labs - tests - and diagnostic studies Prescribes pharmacologic and non-pharmacologic interventions Initiate referrals and consultations to specialty care services Provide education to patients and their families Assess and adjust the plan of care as needed to achieve optimal patient outcomes Exemplify the ICARE core values of-Integrity - Commitment - Advocacy - Respect - and Excellence Influence and model the delivery of evidence-based health care - demonstrate expertise in the ability to manage a patient panel - population - or program Initiate and lead interdisciplinary teams informed by structured approaches and processes to improve outcomes that positively impact care for a patient panel - population - or program Acquire knowledge and skills - then disseminate acquired information within and/or outside the medical facility Initiate and lead interdisciplinary teams developing systems that positively impact outcomes at the panel - population - or program Demonstrate effective communication skills and professional behavior - promoting cooperation and teamwork Meet the needs of customers while supporting the medical center and service missions Communicate and treat customers (patients - visitors - volunteers - and all medical center staff) in a courteous - tactful - and respectful manner Provide the customer with consistent information according to established policies and procedures Handle conflict and problems in dealing with the customer constructively and appropriately Other duties as assigned Benefits VA offers a comprehensive total rewards package: VA Nurse Total Rewards Pay: Competitive salary - regular salary increases - potential for performance awards Paid Time Off: 50 days of paid time off per year (26 days of annual leave - 13 days of sick leave - 11 paid Federal holidays per year) Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions VA Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement) Licensure: 1 full and unrestricted license from any US State or territory Work Schedule: Monday to Friday - 0800-1630 Telework: The Veterans Health Administration – including VISN 19 and the Salt Lake City Health Care System – will comply fully with the intention and instructions contained in the Return to Office Executive Order This position will likely be eligible to telework on a limited Ad-Hoc basis only Virtual: This is not a virtual position Relocation/Recruitment Incentives: Not Authorized Permanent Change of Station (PCS): Not Authorized. #J-18808-Ljbffr
