Job Description
Title: Physical Therapist

Schedule: Mon-Fri

Assignment Details:
Contract Weeks: 13
Guaranteed Hours: 30

Requirements:
- Currently licensed as a Physical Therapist in the state of practice
- Two years of clinical experience preferred; home health experience a plus but not required
- Current CPR certification
- Basic computer proficiency; willingness to learn EMR systems and OASIS documentation

Roles & Responsibilities:
- Provide physical therapy services within the legal scope of practice and licensure requirements.
- Conduct initial evaluations, progress notes, 30-day reassessments, and 60-day recertifications in accordance with regulations and standards.
- Develop, implement, and update individualized plans of care.
- Deliver skilled interventions and supervise assistants in compliance with practice acts.
- Consult with interdisciplinary team members regarding treatment adjustments.
- Educate patients and caregivers on exercise programs and adaptive strategies.
- Prepare and submit assessments and documentation in a timely manner.
- Participate in quality assessment and performance improvement initiatives.

Compliance / Notes:
- Provide care in accordance with infection control, safety, and risk prevention protocols.
- Maintain compliance with privacy and confidentiality standards.
