• Position Type: Contract
• Contract Duration: 3 months
• Pay Rate: $77.00 / Hour (USD)
• Shift / Schedule: Day (5x8 Hours), 8:00 AM – 4:30 PM
• Guaranteed Hours: Facility can call off UP TO one shift per pay period (every two weeks)
• Travel Requirements: Traveler is expected to have their own transportation. Occasional maximum radius of roughly 60 miles (60–90 minute total drive time) per day.
• Work Authorization: Applicants must be authorized to work for ANY employer in the U.S. We are unable to sponsor or take over sponsorship of an employment Visa at this time.

Job Summary

This is a Home Health Physical Therapist travel/contract opportunity serving the East Portland, OR / Clark County, WA and surrounding areas. The role operates on a 5x8-hour day schedule and follows a 1:1 care model, with clinicians visiting up to 6 patients per day at 30 productivity points standard. This facility cannot accommodate 4x10 or 3x12 schedules. First-time travelers are accepted, and local candidates within a 50–75 mile radius are also considered, accepted at the same rate. The system does a good job grouping clients to minimize drive and windshield time.

Key Responsibilities

• Visit up to 6 patients per day in a home health setting; 30 productivity points standard
• Provide skilled home health physical therapy services to patients across East Portland, OR / Clark County, WA and surrounding areas
• Utilize EPIC Remote Client for Home Health (on laptop) and Rover (phone app) / Oasis for charting
• Complete Oasis assessments as required
• Manage independent travel to patient homes, with an occasional maximum radius of roughly 60 miles (60–90 minute total drive time) per day

Required Skills

• Oasis experience required
• Ability to work independently and manage own daily schedule and patient routing
• Valid personal transportation required

Education Requirements

• OR (Oregon) Physical Therapist License — Required
• WA (Washington) Physical Therapist License — Required (Pending license accepted)
• BCLS (Basic Cardiac Life Support) certification — Required

Required Experience

• Minimum 1 year of Skilled Home Health experience required
