Nursing Care Coordinator- Orthopedic Speciality Clinic

Our Fairview Orthopedic Surgery specialists team in Minneapolis, MN provides care for a broad spectrum of bone and joint injuries. We're here to treat conditions affecting ankles, feet, hands, shoulders, elbows, and spine, as well as musculoskeletal tumors.

Location: Fairview Clinic and Surgery Center - 909 Fulton St. SE Minneapolis, MN 55455

Hours: 80 hours/two weeks

Days/Evening shifts

What you will do as an RN Care Coordinator in the clinic:

  • Coordinates care across the health care continuum within designated sub-specialty population(s)
  • Serves as a single point of contact by facilitating and coordinating transitions in care within and between care settings, providers, and services
  • Completes pre-visit planning and post-visit wrap-up and determines vital follow-up appointments and/or treatments to optimize patient visits
  • Facilitates proactive disease management and preventative health care using evidence-based practice
  • Serves as an educator to the patient and medical staff
  • Rooms patients for the surgeon(s) as needed, complete clinic readiness including records review and provides administrative support as needed.

What you will need:

  • Current Minnesota RN license
  • 3+ years of Clinical Nursing experience
  • BLS certification
  • Demonstrated experience in patient advocacy and education, coaching, and proactive health care management along with EMR/digital literacy
  • BSN degree strongly preferred
  • Prefer work experience as either a Clinic Nurse, Care Coordinator Clinic, or Hospital bedside floor nurse patient facing

Compensation Disclaimer

The posted pay range is for a 40-hour workweek (1.0 FTE). The actual rate of pay offered within this range may depend on several factors, such as FTE, skills, knowledge, relevant education, experience, and market conditions. Additionally, our organization values pay equity and considers the internal equity of our team when making any offer. Hiring at the maximum of the range is not typical. If your role is eligible for a sign-on bonus, the bonus program that is approved and in place at the time of offer, is what will be honored.
